The key transition points within education are:

• Nursery to Reception

• EYFS to KS1

• KS1 to KS2

• KS2 to KS3

• KS3 to KS4

Within these transition points, we offer a range of services to support the families, schools and child/young person. Services include:

• Attendance at annual review meetings

• Reports

• Resources e.g. visuals, social stories

• Support to find the appropriate school placement

• EHCP support
